KB Access Patient Experience Survey

The team of providers and support staff at KB Access would like to hear about your experience at the KB Access Clinic. We’re inviting you to take a short, voluntary, and anonymous survey to help us understand what’s working well and where we can improve.

Your responses will support ongoing efforts to enhance the quality of care provided at KB Access. The survey takes about 15 minutes to complete and will run from July 22nd-September 12th 2025.
